Issue 2 described the office administrator post as currently vacant. That was wrong on both counts. Joanne Graham has been the Admin Assistant throughout, and Laura Walden joined on 8 June 2026 as Finance & Compliance Assistant — an additional role, not a replacement. The post was never vacant; the assessment described a vacancy that did not exist while omitting a second person who did. Both are now named as persons at risk. No control, score or action changed — the exposed population is the same and is now described accurately. The review anniversary is unchanged at 04/09/2027.
Setting up and working at a workstation away from the AMWS premises — the Directors working from home on admin days or in the evening, and the Onyx HSQE Consultant working remotely between monthly site visits. Fixed workstations inside the Northampton office are assessed under RA_HO_11 Display Screen Equipment.
Home working locations used by the AMWS Directors and by the Onyx HSQE Consultant. AMWS does not operate a general home-working scheme — the 20 operational teams are field-based and do not home-work.
| Hazard | Existing controls | Initial | Additional controls | Residual | ||||
|---|---|---|---|---|---|---|---|---|
| S | L | R | S | L | R | |||
| Musculoskeletal discomfort from a non-adjustable home setup (dining table, sofa working) | Home working is occasional and short-duration; laptop riser, separate keyboard and mouse available through the Lyreco stationery account; workstation comfort is a standing prompt on FORM_WCC_01 at the weekly Director / HSQE compliance call, so it is asked and recorded rather than left to be raised | 3 | 2 | 6 | Documented DSE self-assessment completed by each home-working individual (Leanne Mason, by 31/10/2026); separate screen, keyboard and mouse provided where home working exceeds one day a week | 2 | 2 | 4 |
| Eye strain and headaches from prolonged screen work | Work is task-varied rather than continuous screen work; no fixed break schedule is set; screen spells are short and broken by calls and site contact. This is stated as a limitation, not claimed as a control | 2 | 3 | 6 | DSE self-assessment to carry the 20-20-20 break prompt; eyesight test on request funded by AMWS under the DSE Regulations — arrangement to be confirmed in writing (Leanne Mason, by 31/10/2026) | 2 | 2 | 4 |
| Controlled documents or customer paperwork held insecurely at home | Controlled documents live on the IMS portal, not in paper; POL_HSQE_09 Data Protection and POL_HSQE_30 IT Security apply to home working | 2 | 3 | 6 | No AMWS paper records to be retained at home; anything printed is returned to the Northampton office for confidential disposal | 2 | 2 | 4 |
| Isolation and reduced day-to-day supervision when working alone at home | Standing weekly Director / HSQE compliance call; WhatsApp Works group; monthly Onyx site visit. Contact cadence is fixed at weekly, monthly and daily rather than left to chance | 2 | 2 | 4 | None required — the existing contact cadence is proportionate for a five-person office and consultant arrangement | 2 | 2 | 4 |
| Poor lighting, heating or ventilation in a space AMWS does not control | Home working is occasional and by choice; the Northampton office is available as an alternative at any time | 2 | 2 | 4 | DSE self-assessment to include a lighting, temperature and ventilation prompt | 1 | 2 | 2 |
| Blurred work / home boundary leading to over-long working days | POL_HSQE_31 Fatigue Management (issued 01/06/2026) applies to office and home-based work as well as field work; Aaron Mason reviews Director working hours at the weekly call | 3 | 2 | 6 | Fatigue self-check prompts under RA_HO_18; no expectation of out-of-hours email response other than a genuine emergency call-out | 2 | 2 | 4 |
This document is maintained under AMWS's continuous-compliance model. Substantive revisions are reviewed and signed off by the Directors at the standing weekly Director / HSQE compliance call (Sean Ashton, Onyx + Leanne Mason). Currency, cross-references and minor edits are checked at the monthly Onyx site visit. The annual Management Review (September) provides the strategic-level confirmation. Compliance is therefore continuous, not gated on a single annual meeting.
