Selection, provision, use, inspection and replacement of personal protective equipment across AMWS operations.
This assessment is numbered in the home and office series but its subject is operational PPE management. Its classification is flagged for the September 2026 Management Review to decide where it properly belongs.
AMWS operational sites, the yard, the yard store and both units.
| Hazard | Existing controls | Initial | Additional controls | Residual | ||||
|---|---|---|---|---|---|---|---|---|
| S | L | R | S | L | R | |||
| PPE not provided to labour-supply subcontractors who are workers rather than employees | Subcontractors provide their own PPE as a condition of engagement; visitor PPE held at the yard. The Personal Protective Equipment at Work (Amendment) Regulations 2022 extend the employer duty to provide PPE to limb (b) workers, not only employees — POL_HSQE_19 was updated on 04/09/2026 to reflect this, but AMWS has not yet worked through which of its labour-supply arrangements fall inside that definition | 3 | 3 | 9 | Review the labour-supply arrangements against the limb (b) worker definition and confirm the PPE provision position for each (Leanne Mason and Sean Ashton, by 31/12/2026). This is an open legal question, not a closed control — the residual stays medium until it is answered | 3 | 2 | 6 |
| PPE issued but not recorded, so condition and replacement cannot be evidenced | PPE issued from the yard store on request and on pre-use inspection failure; a range of sizes is held. Consolidation of the PPE register remains an open gap carried from the May 2026 audit-readiness tracker | 3 | 3 | 9 | Single consolidated PPE register recording issue date, size and replacement due (Leanne Mason, by 31/12/2026). The January 2027 toolbox talk on PPE selection, condition and rejection ties to register sign-off | 2 | 2 | 4 |
| Damaged, expired or unfit PPE remaining in use | Pre-use visual check by the wearer; damaged items replaced from yard stock the same day, with no authorisation required from a Director; POL_HSQE_19 | 3 | 2 | 6 | Rejection criteria — what 'past it' looks like — covered explicitly in the January 2027 toolbox talk; hard hat and harness dates checked at the monthly Onyx site visit | 2 | 1 | 2 |
| Wrong PPE selected for the task | PPE specified in the job RAMS; COSHH assessments specify the PPE required for each substance (APP_09); the Site Supervisor confirms at the pre-start briefing | 3 | 2 | 6 | None — RAMS-driven and COSHH-driven selection is the control and it is working | 2 | 2 | 4 |
| Poor fit, particularly for a smaller or larger wearer | Range of sizes held in the yard store; the wearer chooses their own size | 2 | 2 | 4 | Fit confirmed at the point of issue and recorded on the consolidated register | 2 | 1 | 2 |
| Heat stress from PPE in hot weather | Hydration and break arrangements; summer heat and hydration brief issued June 2026; work planning avoids the hottest part of the day where the programme allows | 3 | 2 | 6 | Work rotation in extreme heat at the Site Supervisor's discretion, with the decision recorded in the job notes | 2 | 1 | 2 |
| Tight-fitting respiratory protective equipment used without face-fit testing | Respiratory protection is specified where a COSHH assessment requires it. No face-fit testing records are held | 3 | 2 | 6 | Confirm whether any AMWS task genuinely requires tight-fitting RPE. Where it does, arrange face-fit testing and record it against the wearer; where it does not, record that finding so the absence of records is explained rather than unexplained (Sean Ashton, by 31/03/2027) | 2 | 1 | 2 |
This document is maintained under AMWS's continuous-compliance model. Substantive revisions are reviewed and signed off by the Directors at the standing weekly Director / HSQE compliance call (Sean Ashton, Onyx + Leanne Mason). Currency, cross-references and minor edits are checked at the monthly Onyx site visit. The annual Management Review (September) provides the strategic-level confirmation. Compliance is therefore continuous, not gated on a single annual meeting.
