Issue 2 carried an open item that Leanne Mason's Emergency First Aid at Work certificate had been rebooked and not filed, with lone-worker first aid cover depending on it. The certificate was produced at the 22/09/2026 site visit, valid to 16/06/2029, and Jason May holds the same qualification to 29/11/2028. The open item is closed and the training requirement names both first aiders. No control, score or action changed. The review anniversary is unchanged at 04/09/2027.
Lone working in the AMWS office and yard context — Leanne Mason alone at the Northampton office during the working day, Aaron Mason or Jason May alone at Unit 2C Stour Road or the yard outside normal hours, and the Onyx HSQE Consultant working alone at a remote location. Lone working on operational sites is controlled through SOP 8.8 and the field risk assessments on the Van Packs portal; the fatigue interaction is assessed in RA_HO_18.
Northampton Office (Unit 1), Unit 2C Stour Road, the yard, and home working locations.
| Hazard | Existing controls | Initial | Additional controls | Residual | ||||
|---|---|---|---|---|---|---|---|---|
| S | L | R | S | L | R | |||
| Accident or sudden illness while alone on the premises with no one to summon help | Mobile phone carried at all times; WhatsApp Works group; the Directors know each other's whereabouts through the working day; HSE 20-person first aid kits held at both premises and checked monthly (APP_18) | 3 | 2 | 6 | Check-out message to the WhatsApp Works group when the last person leaves the premises; anyone working alone at the yard outside 07:00-18:00 tells another Director first | 2 | 1 | 2 |
| No formal lone-worker monitoring for out-of-hours yard or unit work | Arrangements are informal — the Directors are in routine daily contact and know each other's movements. There is no lone-worker device, app or automated check-in in place | 3 | 3 | 9 | Lone-worker check-in app to be trialled and a decision taken on whether to adopt it or to formalise the buddy check-in instead (Sean Ashton, by 30/04/2027). Until that decision is made the residual risk stays medium — the control is a habit, not a system | 3 | 2 | 6 |
| Aggression or intimidation from an unexpected caller at the premises | Both premises are on a secured business estate; external doors kept closed and secure when one person is on site; no cash held on the premises | 3 | 2 | 6 | A lone person does not admit unexpected callers; deliveries taken at the door and not signed for inside; incident reported to the Directors the same day | 2 | 1 | 2 |
| Lone working while fatigued, or at the end of a long site day | POL_HSQE_31 Fatigue Management and the controls in RA_HO_18; Aaron Mason reviews working hours weekly | 3 | 2 | 6 | High-risk or unfamiliar tasks are not undertaken alone at the end of a long day — deferred to the next morning | 2 | 1 | 2 |
| Mental-health impact of prolonged solo working | MHFA cover — Jason May and Leanne Mason, both trained (POL_HSQE_29); standing weekly Director / HSQE compliance call; monthly Onyx site visit; Samaritans 116 123 signposted on the van-pack card | 3 | 2 | 6 | Wellbeing and mental health toolbox talk scheduled April 2027; stress controls held in RA_HO_08 | 2 | 2 | 4 |
| Communication failure — flat phone, no signal | Office landline at the Northampton office as well as mobiles; Wi-Fi at both premises; the Northampton business estate has reliable mobile coverage | 2 | 2 | 4 | None — proportionate to the locations actually used. Satellite or radio equipment would be disproportionate for an urban business estate | 2 | 2 | 4 |
This document is maintained under AMWS's continuous-compliance model. Substantive revisions are reviewed and signed off by the Directors at the standing weekly Director / HSQE compliance call (Sean Ashton, Onyx + Leanne Mason). Currency, cross-references and minor edits are checked at the monthly Onyx site visit. The annual Management Review (September) provides the strategic-level confirmation. Compliance is therefore continuous, not gated on a single annual meeting.
