Issue 2 described the office administrator post as currently vacant. That was wrong on both counts. Joanne Graham has been the Admin Assistant throughout, and Laura Walden joined on 8 June 2026 as Finance & Compliance Assistant — an additional role, not a replacement. The post was never vacant; the assessment described a vacancy that did not exist while omitting a second person who did. Both are now named as persons at risk. No control, score or action changed — the exposed population is the same and is now described accurately. The review anniversary is unchanged at 04/09/2027.
The significance changes here: this assessment records that no documented DSE assessment exists for any workstation, and there are two named, regular DSE users in the office — one of whom has been at a workstation for three months. The action at Leanne Mason for 31/10/2026 was already open; both workstations are now the first it should cover.

| No documented DSE assessment exists for any AMWS workstation | Workstations are conventional desk setups with adjustable office chairs; discomfort is raised informally at the weekly compliance call. No documented DSE assessment is held for any workstation or user — the duty under the DSE Regulations is not currently evidenced | 3 | 3 | 9 | DSE self-assessment completed and recorded for each office workstation and each home-working individual (Leanne Mason, by 31/10/2026), and repeated on any significant change of workstation or user | 2 | 2 | 4 |
| Eye strain and headaches from prolonged screen work | Work is task-varied — screen work is broken up by phone calls, visitors and yard trips; screens positioned to avoid direct window glare where the room layout allows | 2 | 3 | 6 | Eyesight test on request, funded by AMWS as required by the DSE Regulations for DSE users — arrangement confirmed in writing so users know it exists (Leanne Mason, by 31/10/2026). 20-20-20 break prompt carried in the DSE self-assessment | 2 | 2 | 4 |
| Neck, shoulder and back discomfort from laptop-only working | Laptop risers, separate keyboards and mice are available through the Lyreco stationery account | 3 | 2 | 6 | Confirm that each regular DSE user has a separate screen, keyboard and mouse rather than working from the laptop screen, and provide where missing (Leanne Mason, by 31/10/2026) | 2 | 1 | 2 |
| Static posture from prolonged sitting | The role mixes desk work with reception, yard and site contact, so continuous screen spells rarely exceed an hour | 2 | 2 | 4 | Movement and break prompt included in the DSE self-assessment | 2 | 1 | 2 |
| Screen glare and reflection from windows | Screens positioned away from direct window glare where the room layout allows | 2 | 2 | 4 | Glare check included in the DSE self-assessment; screen repositioned or blind fitted where it cannot be designed out | 2 | 1 | 2 |
| Reasonable adjustments not identified for a DSE user with a health condition | POL_HSQE_29 commits AMWS to considering practical adjustments; the Equality Act 2010 duty is recognised in POL_HSQE_11 | 3 | 2 | 6 | The DSE self-assessment invites the user to raise any condition affecting their DSE work, handled confidentially by a Director and documented only with the user's consent | 2 | 1 | 2 |
This document is maintained under AMWS's continuous-compliance model. Substantive revisions are reviewed and signed off by the Directors at the standing weekly Director / HSQE compliance call (Sean Ashton, Onyx + Leanne Mason). Currency, cross-references and minor edits are checked at the monthly Onyx site visit. The annual Management Review (September) provides the strategic-level confirmation. Compliance is therefore continuous, not gated on a single annual meeting.
