Issue 2 described the office administrator post as currently vacant. That was wrong on both counts. Joanne Graham has been the Admin Assistant throughout, and Laura Walden joined on 8 June 2026 as Finance & Compliance Assistant — an additional role, not a replacement. The post was never vacant; the assessment described a vacancy that did not exist while omitting a second person who did. Both are now named as persons at risk. No control, score or action changed — the exposed population is the same and is now described accurately. The review anniversary is unchanged at 04/09/2027.
Prevention of slips, trips and falls on the AMWS premises — office floors, the yard, the yard store and the car park and approaches. Slips and trips in excavations and on operational sites are controlled through the field risk assessments on the Van Packs portal.
Northampton Office (Unit 1), Unit 2C Stour Road, the yard, the yard store and the car park and approaches.
| Hazard | Existing controls | Initial | Additional controls | Residual | ||||
|---|---|---|---|---|---|---|---|---|
| S | L | R | S | L | R | |||
| Wet floor at entrances in wet weather | Entrance matting at the units; footfall is low and the wet area is small; no cleaning contractor attends during the working day, so a spill is cleared immediately by the person who finds it — mop and absorbent granules are held at each unit for the purpose | 2 | 3 | 6 | Matting condition checked as a recorded item at the monthly Onyx site visit; spill-wiping expectation made explicit rather than assumed | 2 | 2 | 4 |
| Trailing cables in the office | Cables routed to the wall behind desks; only a small number of fixed workstations | 2 | 2 | 4 | None — covered by the workstation layout prompt in the DSE self-assessment under RA_HO_11 | 2 | 2 | 4 |
| Ice on the yard, the car park and the unit approaches in winter | Shared estate access roads are the estate management's responsibility. AMWS's own approaches and yard are dealt with by the first person on site. No grit stock or named responsibility is currently recorded | 3 | 3 | 9 | Bagged grit or a grit bin held at the yard for the AMWS approaches and yard, with responsibility for treating them named (Jason May, by 30/11/2026, ahead of winter). Reinforced by the December 2026 cold-weather toolbox talk | 2 | 2 | 4 |
| Uneven or damaged yard surface | Yard surface inspected at the monthly Onyx site visit, with defects recorded on the site visit report and carried to the Directors at the weekly compliance call until closed | 3 | 2 | 6 | Yard surface defects logged with an owner and a target date rather than reported verbally, so repairs can be evidenced | 2 | 1 | 2 |
| Poor lighting in the yard on winter early starts and late finishes | External lighting at both units; emergency lighting inside on a monthly test and annual service cycle to BS 5266 (APP_18) | 3 | 2 | 6 | External lighting checked as part of winter preparation before the clocks change (Jason May, by 30/11/2026) | 2 | 1 | 2 |
| Cluttered walkways and floor-level storage in the yard store | Housekeeping standards applied by the Site Supervisor; yard store housekeeping checked at the monthly Onyx site visit | 2 | 3 | 6 | Walkways kept clear and stock kept off the floor | 2 | 2 | 4 |
| Slip or trip while carrying materials or stock | Route cleared before carrying; loads split so the carrier can see ahead (RA_HO_07) | 3 | 2 | 6 | Controls held in RA_HO_07 rather than duplicated here | 2 | 1 | 2 |
This document is maintained under AMWS's continuous-compliance model. Substantive revisions are reviewed and signed off by the Directors at the standing weekly Director / HSQE compliance call (Sean Ashton, Onyx + Leanne Mason). Currency, cross-references and minor edits are checked at the monthly Onyx site visit. The annual Management Review (September) provides the strategic-level confirmation. Compliance is therefore continuous, not gated on a single annual meeting.
