| Hazard | Existing controls | Initial | Additional controls | Residual | ||||
|---|---|---|---|---|---|---|---|---|
| S | L | R | S | L | R | |||
| A subcontractor works unsafely on an AMWS job | RAMS issued and signed before work starts; method statements and field risk assessments published on the Van Packs portal; Jason May supervises; work only allocated to firms and individuals on the approved list held in APP_19 | 4 | 2 | 8 | FORM_PTW_01 permit to work used for excavation, hot work and confined space entry, with the close-out captured as the reference example (open action from IA202604) | 3 | 1 | 3 |
| Subcontractor competence not verified before deployment | Tickets and qualifications tracked in Leanne Mason's live register, referenced by APP_13; EUSR Water Hygiene and NRSWA cards checked before a person works on a job | 4 | 2 | 8 | Combined operator-equipment competency matrix (CAR-2026-004, target 30/09/2026) to give a single view rather than two separate registers | 3 | 1 | 3 |
| Inadequate or lapsed subcontractor insurance | Insurance checked at engagement under SOP 5.3; suppliers and subcontractors reviewed through the APP_19 Approved Supplier Register | 3 | 2 | 6 | Insurance expiry dates captured in the approved supplier register with a renewal prompt, so a lapse is visible before it happens rather than at the next annual review | 2 | 1 | 2 |
| Right to work not verified for a labour-supply subcontractor | PROC_R2W_01 Right to Work procedure; POL_HSQE_17 Illegal Workers; checks held by Leanne Mason | 4 | 2 | 8 | Annual right-to-work sweep with a calendar entry so it happens on a date rather than on memory (CAR-2026-011, Leanne Mason, target 30/04/2027) | 3 | 1 | 3 |
| Poor communication at the interface between AMWS teams and subcontractors | Site briefing before work starts; WhatsApp Works group for operational cascade; Jason May is the single point of contact for site matters | 3 | 2 | 6 | None — a single named point of contact is the proportionate control for a team of this size | 2 | 2 | 4 |
| A contractor attending the AMWS premises works unsafely or is unaware of the emergency arrangements | Visitors and contractors signed in; escape routes and assembly point pointed out; work supervised by the Director or Site Supervisor on the premises; contractors are not left to work unaccompanied | 3 | 2 | 6 | Visiting-contractor briefing recorded at sign-in rather than given verbally and not evidenced | 2 | 1 | 2 |
| Subcontractor performance affecting delivered quality | Work inspected before sign-off; APP_19 annual supplier review with the four checks answered from AMWS records; poor performers moved to inactive | 2 | 2 | 4 | None — the existing review cycle is proportionate to the size of the supply base | 2 | 2 | 4 |
This document is maintained under AMWS's continuous-compliance model. Substantive revisions are reviewed and signed off by the Directors at the standing weekly Director / HSQE compliance call (Sean Ashton, Onyx + Leanne Mason). Currency, cross-references and minor edits are checked at the monthly Onyx site visit. The annual Management Review (September) provides the strategic-level confirmation. Compliance is therefore continuous, not gated on a single annual meeting.
