The document

Purpose

This register is the central index of every occupational health and safety risk assessment held by A M Water Services Limited, so that assessments are tracked, reviewed and maintained under ISO 45001 Clause 6.1.2 and Regulation 3 of the Management of Health and Safety at Work Regulations 1999.

Scope

All 18 OHS risk assessments, covering the premises, the yard, home and remote working, information and cyber, people and wellbeing, and operational and external activities. Each row carries the statutory basis for the assessment on the same line as the assessment itself. A separate compliance matrix used to hold that mapping and had fallen out of step with the register; folding it in removed the second copy.

Key to the review status

Derived from the next-review date against 27/09/2026, not typed. The lifecycle state, Active or otherwise, is a separate column, because an assessment can be current in its lifecycle and out of date on its review.

ValueHow it is decided
In dateNext review is more than 90 days away.
Due within 90 daysNext review falls inside the next 90 days.
OverdueNext review has passed. Treated as a gap, not as compliance.
No date heldNo next-review date is recorded against the assessment.

Quality control of the control statements

A control written as "regular checks" or "as required" cannot be audited, cannot be measured and gives a worker nothing to follow. Every control statement in every assessment is tested against six questions, and passes only if the answer to all six is yes or the question does not apply.

#TestThe question it answers
1NamedDoes the control name the document, record, equipment or person that delivers it, rather than describing an intention?
2Frequency statedIf the control recurs, does it say how often — daily, weekly, monthly, annually, pre-use?
3EvidencedIs the record that demonstrates the control available today?
4OwnedWhere the control is an action still to be taken, is a named individual accountable?
5DatedWhere the control is an action still to be taken, does it carry a completion date?
6HonestWhere a control is not in place, does the assessment say so plainly rather than implying it is?

QC result — 9 September 2026

All 18 assessments and all 120 control statements were tested by the HSQE Consultant as the closing step of the September 2026 review, over the same population reissued on 04/09/2026.

MeasureResult
Assessments in scope18

Passed all applicable tests at first pass102 (85%)
Failed test 1, 2 or 3 and were rewritten16 (13%)
Failed no test but state an absent control deliberately2 (2%)
Failing statements remaining after rewrite0

Tests 4 and 5 pass across the whole population: every outstanding action in every assessment carries a named individual and a completion date, consolidated into the action table in APP_07.

Test 6 passes by design. The September review replaced controls that earlier issues had claimed but that did not exist — DSE assessment forms, an ergonomic equipment voucher scheme, PAT-tested equipment provision, annual home safety assessments, an Employee Assistance Programme and a salary advance scheme. Each is now stated in bold as absent in the assessment that would otherwise rely on it.

Two statements use the word informal and were left as written, because both describe an absent control rather than claim a present one, which is what test 6 requires: RA_HO_04 out-of-hours lone working, where a written check-in arrangement is open to Sean Ashton for 30/04/2027, and RA_HO_11 display screen equipment.

The sixteen rewrites tightened how controls are described. None changed what is controlled, added a control or altered a risk score, so the assessments remain at Issue 2, 04/09/2026 and this section is the record of the QC. Any future change to a control itself carries a new issue number in the normal way.

Review requirements

Annually as a minimum. The review considers:

Incident history
Near-miss reports
Changes in work practices
New equipment or technology
Feedback from workers
Audit findings

Outside the annual cycle, a review is triggered immediately by:

Serious incident or near miss
Significant change in process
New hazard identified — HO-18 Fatigue was added on 04/05/2026
Regulatory update
Audit nonconformity
Worker concern raised
