Issue 3, 28/09/2026 โ€” a second qualified first aider is evidenced

Issue 2 recorded first aid cover as resting on Leanne Mason alone, with her rebooked Emergency First Aid at Work certificate committed but not filed. Both points are now evidenced. Leanne Mason's certificate was produced at the 22/09/2026 site visit โ€” Highfield Level 3 Award in Emergency First Aid at Work (RQF), awarded 16/06/2026, valid to 16/06/2029. Jason May holds the same qualification, awarded 29/11/2025 and valid to 29/11/2028, so AMWS has held two qualified first aiders throughout. The residual on the first-aider line falls from ๐ŸŸก 8 to ๐ŸŸข 4. The review anniversary is unchanged at 04/09/2027.

No in-date qualified first aider available when an injury happensTwo qualified first aiders, both certificates on file. Leanne Mason and Jason May each hold the Highfield Level 3 Award in Emergency First Aid at Work (RQF), qualification 610/0759/6, delivered by Clancy Group โ€” Leanne Mason awarded 16/06/2026 and valid to 16/06/2029, Jason May awarded 29/11/2025 and valid to 29/11/2028. Cover no longer depends on one individual being present428Both first aiders are named in the induction and on the first aid notices at each premises, so anyone on site knows who to call (Leanne Mason, by 31/10/2026)414
First aid kit contents expired or depleted when neededFive kits held โ€” one burns kit and one HSE 20-person workplace kit at the premises, three vehicle kits in the fleet. Inspected quarterly with the inspector named, seal and contents checked, and the result recorded in APP_18. The record runs to 22 inspections with no interval longer than 92 days313None required โ€” the quarterly recorded inspection is a working control and the evidence trail is complete313
Delayed emergency response at a site with no clear postal addresswhat3words used to give the emergency services a precise location; vehicle first aid kits carried; mobile coverage across the operating area428what3words reference recorded in the RAMS for any job without a street address, so it is ready before it is needed rather than looked up under pressure (Jason May)313
No defibrillator available for a cardiac arrestAMWS holds no AED. The APP_18 inspection-frequency table lists a defibrillator on a weekly visual and annual service cycle, but no AED appears in the asset register โ€” that row describes an intention, not an asset. The nearest public-access defibrillator location is not currently recorded428Decide whether to purchase an AED for the Northampton premises. In the meantime record the nearest public-access defibrillator location for both units, and correct the APP_18 frequency table so it does not imply equipment AMWS does not hold (Sean Ashton, by 31/12/2026)326
Cross-contamination during first aid treatmentDisposable gloves, face shields and resuscitation aids held within the kits; hygiene covered in first aid training224None โ€” kit contents are to HSE specification and are checked quarterly224
No psychological first aid after a traumatic incidentMHFA cover โ€” Jason May and Leanne Mason (POL_HSQE_29); Samaritans 116 123 on the van-pack signposting card. No Employee Assistance Programme is in place โ€” see RA_HO_08326Post-incident support pathway written into the incident procedure so it is triggered rather than remembered (Sean Ashton, by 31/03/2027); the EAP decision remains open under RA_HO_08224
First aid provision not matched to actual needKit provision is at HSE 20-person scale, which suits the headcount, and kits are placed at both premises and in the fleet. No documented first aid needs assessment is held326Document a first aid needs assessment covering office, yard and field work โ€” hazards, headcount, travel distances and lone working โ€” and use it to set the number of trained first aiders required (Sean Ashton, by 31/12/2026)212

How this document is approved

This document is maintained under AMWS's continuous-compliance model. Substantive revisions are reviewed and signed off by the Directors at the standing weekly Director / HSQE compliance call (Sean Ashton, Onyx + Leanne Mason). Currency, cross-references and minor edits are checked at the monthly Onyx site visit. The annual Management Review (September) provides the strategic-level confirmation. Compliance is therefore continuous, not gated on a single annual meeting.
