Positive observations

  • Site set-up to NRSWA standard: TM in place per chapter 8, edge protection rigid, signed approach
  • Daily briefing record signed by all four operatives at start of shift (RAMS-5.1)
  • Excavation correctly battered, spoil set back >1m from edge
  • Spill kit on site, located by the bowser, lid intact
  • All operatives in correct PPE for hot-work-adjacent zone (Lapwing site boots, Hi-Viz Class 2, helmets)
  • Welfare unit clean, hand-wash facility working, kettle PAT-tested

Concerns / unsafe acts / unsafe conditions

  • One operative observed during 10:00 break without gloves while handling a length of pipe being moved into the spoil pile. Spotted by Aaron, corrected on the spot — operative reminded of the standing rule that gloves are worn whenever handling pipe / fittings regardless of break status.

Worker engagement (questions asked / feedback received)

  • Site Supervisor (Jason May) asked about the new 2026 SOP HTML format on the portal — confirmed easier to find than the old SVGs. Aaron noted feedback for next IMS update.
  • Operative raised concern about the wear rate on Lapwing gloves on aggregate-handling shifts — agreed to spec a heavier-duty glove option for next PPE order.
