Read the zeros carefully

The zeros in the table above are two different things.

Zero accidents is a genuine achievement. Three years of excavation, confined-space and highway working across twenty field teams have produced no RIDDOR reportable, no lost-time injury, no dangerous occurrence and no road traffic accident.

Zero near misses is a failure of reporting, not an absence of events. Teams working in trenches, on live carriageways and around live mains generate near misses. None have been reported, so the business has no early-warning data.

Objective H4 — the reporting route is not being used

No numeric target has been set for recovery. Setting one before the cause of zero reporting is understood would produce reports written to hit a number. The objective is owned by Sean Ashton and Jason May and tracked in APP_11.

Reporting process

  1. Notify your supervisor immediately. Do not wait until end of shift.
  2. Complete an Incident Report the same day. See SOP 8.1 — Accident, Incident & Near Miss Reporting.
  3. For accidents, complete an Accident Log entry within 24 hours.
  4. For near misses, complete a Near Miss Record. The form is HS_FM_03 on the AMWS forms portal.
  5. Investigations are conducted by the HSQE Coordinator and documented as Investigation Reports.

Reporting a near miss carries no blame and no consequence for the reporter.
