Statement of Nonconformity: OBSERVATION: Near-miss reporting rate below target KPI of 10 per month. While no incidents have occurred, increased near-miss reporting would provide more learning opportunities.

  2. Implement monthly recognition programme (01/12/2025)
  3. Share near-miss learning in toolbox talks (Ongoing from December)

Root Cause

Primary Cause: Cultural perception that near-misses reflect negatively on teams  Contributing Factors: Lack of positive reinforcement for reporting

Proposed Corrective Action

  1. Monthly "Safety Champion" recognition for best near-miss report.
  2. Emphasise learning value.
  3. Remove any negative associations with reporting.

Verification

  1. [ ] Achievement of 10+ near-miss reports per month for 3 consecutive months.
