Statement of Nonconformity: OBSERVATION: Near-miss reporting rate below target KPI of 10 per month. While no incidents have occurred, increased near-miss reporting would provide more learning opportunities.

Proposed Corrective Action¶
- Monthly "Safety Champion" recognition for best near-miss report.
- Emphasise learning value.
- Remove any negative associations with reporting.
