Assessment Methods¶
The two assessment families in the system — occupational health and safety, and substances hazardous to health — and the one scoring matrix they share.
| Framework Details | |
|---|---|
| Risk assessments | 18, listed at Risk Assessments |
| COSHH assessments | 30, listed at COSHH |
| Scoring | One 4 × 4 matrix, applied everywhere |
| Controlled By | Sean Ashton (HSQE Consultant) |
| Approved By | Aaron Mason, Managing Director |
One matrix, applied everywhere¶
The same 4 × 4 severity × likelihood matrix is used in every risk assessment, in the hazard log and in the OHS register, so a score means the same thing wherever it appears.
| Factor | 1 | 2 | 3 | 4 |
|---|---|---|---|---|
| Severity | Negligible | Minor | Serious | Catastrophic |
| Likelihood | Remote | Unlikely | Likely | Almost Certain |
| Rating | Score (S × L) | Action | ||
| -------- | --------------- | -------- | ||
| Low | 1–5 | Monitor and maintain controls | ||
| Medium | 6–11 | Additional controls, regular review | ||
| High | 12–16 | Immediate action, Director attention |
The two methods¶
| Method | What it covers |
|---|---|
| Risk Scoring Method | How occupational health and safety risks are identified, scored and controlled, and how residual risk is judged acceptable |
| COSHH Assessment Method | How substances hazardous to health are assessed — classification, exposure, control selection and review |
The methods define how the matrix is applied. The assessments themselves apply it, and are listed once on their own index pages.
