Details of Nonconformity

Statement of Nonconformity: OBSERVATION (rolled forward from CAR-2025-005 / -016): APP_12 Training Matrix and APP_14 Calibration & Equipment Log remain separate registers. A combined operator-equipment competency view linking specific operator qualifications to specific equipment authorisations would strengthen clause 7.2 evidence. Lower priority for the B2 audit; a 2026–27 IMS improvement candidate.

Root Cause

Primary Cause: Operator competence and equipment authorisation are tracked in two separate registers; no combined view yet exists. Contributing Factors: Small operative team — the link is currently held informally.

Proposed Corrective Action

  1. Build a single matrix mapping each operative's qualifications to the equipment they are authorised to operate.
  2. Decide whether this lives in APP_12, APP_14, or the new QualTrack tracker.

Verification

  1. [ ] Combined operator-equipment competency matrix exists and is populated.
  2. [ ] Sample operatives' equipment authorisations trace to a held qualification.

Recommendations

  1. Fold this into the QualTrack rollout rather than maintaining a third spreadsheet.
  2. Treat as a 2026–27 improvement — not required for the B2 surveillance audit.
