The headline, stated plainly

Reactive performance is excellent. Proactive performance is not working.

Across three years of excavation, confined-space and highway working, twenty field teams have recorded zero RIDDOR reportables, zero lost-time injuries, zero dangerous occurrences and zero road traffic accidents.

Against that, near-miss and close-call reporting has stood at zero for three years. Teams working in trenches, on live carriageways and around live mains generate near misses. None have been reported, so the reporting route is not being used. It is objective H4 in APP_11, owned by Sean Ashton and Jason May. The full analysis is in APP_22.
