Executive Summary

Mental Health Awareness Week 12-18 May; interim Management Review issued; BCP desktop exercise 28 May; three pre-audit TBTs land before the Achilles UVDB audit on 3-4 June.

🔴 Achilles UVDB B2 Audit — 3-4 June 2026

We are four weeks out. Status going in:

  • IMS portal fully refreshed (13 May rebuild) — 23 appendices, 18 RAs, 16 internal audits all on consistent template
  • Master B2 Evidence Pack current; all 13 category folders populated
  • 23 gap-closure items: 18 closed, 5 in active progress for May
  • Cloudflare auditor onboarding ready once Achilles confirms the auditor's email

On the day: Leanne leads, Sean on-call as HSQE support, Aaron on-site for opening/closing. Everyone else: business as usual, ready for site interview if requested.

Interim Management Review — Issued 13 May

The mid-year interim Management Review (Issue 4 of FORM_MR_001) is now issued and lives at audits/management-review/2026/. Headline outputs:

  • IMS substantively rebuilt; all 23 appendices and 18 RAs on consistent template
  • CAR-2026-001..011 all dispositioned with auditor-binary verdicts (Closed with rationale or Open — rolled forward)
  • Director Site-Tour programme retired; replaced by AMWS H&S Culture Survey (Onyx Operations) rolling out Q3 2026
  • 2025 Carbon Baseline published — 288.7 tCO₂e Scope 1+2; diesel is 97% of footprint

Full annual MR follows in September 2026 once the Achilles outcome and Q3 work is in.

Mental Health Awareness Week — 12-18 May

This year's MHAW theme is community. Construction has the highest male-worker suicide rate of any UK industry — community is the protective factor.

AMWS this week:

  • Tuesday 13 May — yard breakfast and chat (Aaron + Leanne)
  • Mid-week Lighthouse helpline (0345 605 1956) card recirculation in every van pack
  • Jason and Leanne both MHFA-trained — door is always open
  • POL_HSQE_29 Mental Health is the policy backing

BCP Desktop Exercise — Thursday 28 May

The Business Continuity Plan (APP_17) is being tested in a desktop exercise covering Scenario #3 — Site Supervisor (Jason May) unavailable for 5+ working days:

  • First-24-hour response — Aaron deputises as Site Supervisor; morning briefings, site visits and RAMS approvals walk-through; Sean covers scheduled TBT delivery
  • Beyond 24 hours — recovery path including freelance Site-Supervisor cover via the existing network

Attendees: Sean (facilitator), Aaron, Leanne. Outputs feed CAR-2026-005 closure and the audit-readiness evidence.

HSE Prosecution — Reminder on Plant Operation

A recent HSE case (Hampshire, early 2026) involved a worker who lost both legs after being crushed by a 15-tonne excavator. Site manager was also fined for obstructing inspectors.

The AMWS rule, always: when plant is moving, nobody is in the plant's working radius. Banksman with line-of-sight, PPE high-vis on, and the operator has the right of refusal if anything is unclear.
