Details of Observation

Statement of Observation: The September 2025 Management Review action to implement QR coding for plant equipment (original target 31/01/2026) did not complete. The intent — scannable identification tying each item of plant to its inspection, calibration and maintenance record — remains sound, but it now depends on the e-forms workstream rather than standing alone. The action is closed as a 2025 item and re-raised here so that it carries a current owner, a realistic target and a visible dependency.

Planned Actions

Containment Action(s), including correction, with supporting completion date(s):

  1. Existing plant identification and the APP_14 calibration record remain the control in the interim (no gap in inspection evidence)
  2. QR coding formally scoped as part of the e-forms workstream rather than as a separate initiative

Root Cause

Primary Cause: The action was set without a delivery mechanism behind it; QR coding only becomes useful once there is a digital form to scan through to. Contributing Factors: The e-forms proposal was itself only committed to as a drafted IMS document by 30/09/2026 at the May 2026 review.

Proposed Corrective Action

  1. Confirm the e-forms scope and whether plant QR coding is inside it (by the September 2026 Management Review).
  2. Where it is in scope, specify which plant items are tagged and what each tag resolves to.
  3. Where it is not, close this CAR with a documented rationale rather than rolling it forward a third time.

Verification

  1. [x] The QR-coding position is stated either way — not within the e-forms scope. The business forms portal at forms.amwaterservices.co.uk covers business forms. Plant tagging is not part of it.
  2. [x] Decision recorded here and carried as an input to the next Management Review.

Points noted

  1. Do not roll this forward again without either a delivery date or a decision to drop it. A third rollover without a decision is the point at which the action stops being managed.
  2. If it proceeds, start with the plant that already carries a statutory inspection interval, where the payback is clearest.

Feedback & Acknowledgments

Raised at the May 2026 interim Management Review and formally logged on 01/06/2026. Originates in a September 2025 Management Review action, converted to a corrective action at this review so that it carries a named owner, a target date and a verification route. Owner and target agreed by the Directors at the meeting.

Closure

Closed with the rationale the third action called for, rather than rolled forward again. Plant QR coding is not within the e-forms scope. The underlying requirement — each item of plant identified and tied to its inspection, calibration and maintenance record — is met by the Kit & Equipment Log and the calibration records held by the admin team, which are the masters for that information. QR tagging would be a faster route to records that already exist and is not required by any clause, legal duty or client condition.

The intent is recorded as not proceeding. If it is wanted later it comes back as a planned improvement with its own scope and date, not as a corrective action.

Status: CLOSED 05/10/2026 — not proceeding; plant identification and its records are held in the Kit & Equipment Log and the calibration records
