About this record
Convened under the interim provision at SOP 6.4 to review the classification of three corrective action reports raised by the management system reviews of September 2026, and to carry forward the conclusion that followed from them.
Single-topic review. The Clause 9.3.2 input and the Clause 9.3.3 outputs are both held in this document. The September 2026 annual record stands as issued on 29/09/2026; this review records a later decision and does not amend it.
1. Input considered¶
| Input | Source |
|---|---|
| Classification of CAR-2026-015, CAR-2026-016 and CAR-2026-017 | APP_21 Corrective Action Register |
| The definition applied: non-fulfilment of a requirement | ISO 9000 · ISO 9001 Clause 10.2 |
| Position of each matter at the date of this review | APP_12, APP_24, the personnel records and the business forms portal |
2. Discussion¶
The September 2026 reviews recorded three reports as nonconformities. Each was tested at this review against the single question the standard asks: is there objective evidence that a specified requirement was not fulfilled?
CAR-2026-015 — statutory certificates lapsed after an earlier corrective action closed. A Fire Marshal certificate reached expiry and a first aid certificate accepted at closure as a commitment had not been filed. A requirement was not fulfilled, and the position persisted beyond the cycle in which it arose. Classification confirmed: nonconformity. One certificate renewal remains open against it.
CAR-2026-016 — the two office administrators and the register entries. Both administrators were in post and properly onboarded, with the signed acknowledgement, the induction paperwork and the training record held on the personnel file throughout. What lagged was the transcription of that onboarding into the registers, together with wording introduced during a risk assessment reissue that described a post as vacant. Both were identified and corrected inside the same review cycle, and nothing was relied upon in the interval. No requirement was left unfulfilled. Reclassified: withdrawn.
CAR-2026-017 — the business forms portal. Leanne Mason introduced forms.amwaterservices.co.uk as the portal for the company's business forms. The forms it holds were brought on to APP_24 within the same review cycle, and APP_24 is the register of record for them. No requirement was left unfulfilled. Reclassified: withdrawn.
The distinction drawn. A matter found and put right within the cycle in which it arises, where nothing was relied upon in the interval and no requirement was left unfulfilled, is the management system working. It is recorded as work done. A nonconformity is recorded where a requirement was not met and the system did not catch it in time to prevent that.
3. Conclusion¶
The conclusion recorded at the September 2026 annual review — that a clean internal audit result alongside three nonconformities showed the 2026 sample had not reached where the faults lay — rested on three. One stands. CAR-2026-015 arose in September 2026, after the April–May audit cycle had closed, so it was not available to that cycle to find.
The Directors therefore do not draw an adverse conclusion about the 2026 internal audit sample. The rescoping of the 2027 cycle to test records against registers is nonetheless retained on its own merit: testing whether the people, forms and certificates the system claims exist actually do is a better test than reading documents against their own wording, whatever the 2026 result.
4. Outputs and decisions — Clause 9.3.3¶
| # | Decision |
|---|---|
| 1 | CAR-2026-015 confirmed as a nonconformity. Open against the outstanding Fire Marshal renewal, target 31/10/2026. |
| 2 | CAR-2026-016 withdrawn. The references are retained so they are not reissued. |
| 3 | CAR-2026-017 withdrawn on the same basis. |
| 4 | APP_21 reissued at Issue 10 to carry the withdrawals and the basis for each. |
| 5 | The September 2026 conclusion on internal audit effectiveness is superseded by Section 3 above. The 2027 rescoping stands. |
| 6 | SOP 7.1 to be reviewed so that a corrective action report is raised where a requirement has not been fulfilled, and matters put right within the cycle are recorded as work done rather than as findings. |
5. Actions arising¶
| # | Action | Owner | Target |
|---|---|---|---|
| 1 | Issue this record | Sean Ashton | 05/10/2026 |
| 2 | SOP 7.1 reviewed and reissued | Sean Ashton | 30/11/2026 |
