Details of Observation¶
Statement of Observation: The September 2025 Management Review action to develop training modules through Onyx Operations (original target 31/03/2026) did not complete as specified. The underlying need — structured, repeatable awareness content — has been met operationally by the TBT_PROG_01 twelve-month rolling toolbox-talk programme issued 04/05/2026, which carries a topic per month tied to the relevant SOP and policy. What has not been settled is whether a separate modular training product is still wanted on top of that.

Root Cause¶
Primary Cause: The original action described a deliverable without defining its scope, audience or the gap it filled beyond the toolbox-talk layer. Contributing Factors: The TBT programme delivered much of the practical benefit, which reduced the urgency without formally changing the action.
Proposed Corrective Action¶
- Define what a training module would add beyond TBT_PROG_01 — induction content, role-specific competence, or refresher assessment.
- Where the addition is real, scope and target it.
- Where TBT_PROG_01 covers the need, close this CAR with that rationale recorded.

Points noted¶
- Test the question against the training matrix (APP_12) — if a competence requirement has no delivery mechanism behind it, that is the module worth building.
- Consider whether this belongs with the ComplianceTrack qualification data rather than as a separate workstream. CAR-2026-004, which carried the combined operator-equipment view, was closed on 29/09/2026 as raising no nonconformity.

Raised at the May 2026 interim Management Review and formally logged on 01/06/2026. Originates in a September 2025 Management Review action, converted to a corrective action at this review so that it carries a named owner, a target date and a verification route. Owner and target agreed by the Directors at the meeting.

Closure¶
Closed with the rationale the third action called for. The need the September 2025 action described — structured, repeatable awareness content — is delivered by TBT_PROG_01, the twelve-month rolling toolbox-talk programme, which carries a topic per month tied to the relevant SOP and policy and now runs November 2026 to October 2027. Tested against APP_12, every competence requirement on the matrix has a delivery route behind it.
No separate modular training product is required, so none is being scoped. If a competence requirement appears later without a delivery route, that is the module worth building and it comes back with its own scope and date.
Status: CLOSED 05/10/2026 — need met by TBT_PROG_01; no separate training module being built
