Review history

Year Review Date Type Issue Status
2026 Management Review — September 2026 (forthcoming) 30/09/2026 (planned) Annual 5 Planned
2026 Management Review — May 2026 (interim) 13/05/2026 Interim 4 Issued
2025 Management Review — September 2025 27/09/2025 Annual 3 Archived

Cadence

  • Annual Management Review — September each year, paired with the surveillance audit cycle and the year-end financial close. Strategic-level review.
  • Interim Management Review — convened when substantive IMS change warrants leadership sign-off (e.g. a major rebuild, a new external audit window). Captures position at the time so the annual review can focus on year-on-year trend.
  • Continuous-compliance model — the standing weekly Director / HSQE compliance call + monthly Onyx site visit are the substantive review forums between formal MRs. The annual MR provides strategic confirmation, not the gating event.

Historical MRs are retained verbatim — never edited after issue — so the auditor can trace the strategic-decision audit trail year-on-year.

How this section is approved

Management Review records are reviewed and approved by the Directors (Aaron Mason + Leanne Mason) at the meeting itself; the HSQE Consultant (Sean Ashton, Onyx Operations) prepares the agenda and minutes. Once issued, MR records are archived and not amended — substantive change is captured in the next MR.
