Type: ☐ Internal Audit ☐ Client Audit ☐ Inspection ☐ Complaint ☐ Sub-contractor ☑ Certification Review
Classification: ☐ Nonconformity ☑ Observation ☐ Opportunity for Improvement
Details of Observation¶
Statement of Observation: Raised by Alcumus ISOQAR at the initial certification audit of 19–22 November 2024, recorded as OBS1: "Small racking seen in use and not overloaded at the time of visit, however it might benefit to record the SWL and conduct regular inspections."
This report brings that observation onto the corrective action register. It was previously tracked on an improvement activity form, which is why it does not appear in the 2025 series alongside the internal audit findings of the same period. The observation is recorded at the date it was raised; this report is dated when it entered the register.
Planned Actions¶
Containment Action(s), including correction, with supporting completion date(s):
- Racking load capacity assessed and safe working loads determined (15/12/2024)
- Safe working load signage installed on all racking, with visual management (31/01/2025)
- Documented inspection regime established for the racking (31/01/2025)
Root Cause¶
Primary Cause: The racking was in service without a loading assessment because nothing in the system required one. Racking was treated as fixtures rather than as work equipment, and the equipment register covered plant and tools. Nothing connected the storage racking to an inspection interval or a stated load limit. Contributing Factors: The stores were small enough that loading was managed by judgement on the day and had never caused a problem, so the absence of a written limit was never forced into view.
Proposed Corrective Action¶
- Determine and mark the safe working load for each racking run.
- Bring the racking into a documented inspection interval rather than relying on judgement at the point of use.
- Reflect the stores and yard arrangements in the home-and-office risk assessments so the control has a home in the system rather than on a project form.
Verification¶
- [x] Safe working loads determined and displayed on the racking.
- [x] Racking condition and loading checked at the monthly Onyx site visit, with the stores and yard covered in RA_HO_07 and RA_HO_10.
- [x] Stock held between knee and shoulder height, with only empty packaging stored above shoulder height.
Points noted¶
- External audit findings are raised on this register in the same way as internal ones, using the Certification Review type box on this form. This report is the first to use it.
- The status of any observation raised by the certification body is confirmed with them at the following visit rather than closed unilaterally.
Feedback & Acknowledgments¶
Raised by Alcumus ISOQAR at the initial certification audit, 19–22 November 2024. The work was carried out by Leanne Mason, Director, between November 2024 and January 2025.

Closure¶
The action was completed in January 2025. This report records it on the register.
The observation was addressed at the time: loads were assessed, safe working loads were determined and displayed, and the racking was brought into the monthly site inspection rather than left to judgement at the point of use. It was tracked on an improvement activity form through to completion.
Why it is recorded here. The improvement activity forms from the initial certification cycle were withdrawn on 29/09/2026 as delivered, so the finding was transcribed onto this register, which is where findings raised by the certification body are held.
Confirmation with Alcumus ISOQAR. OBS1 was marked for closure at the first surveillance visit. This report is closed on the evidence of the work done; formal confirmation of an observation raised by the certification body follows its own surveillance cycle.
