Programme, method and register of cycles: APP_27 — HSQE Culture Audit Register

A director must be present for the whole audit

That is not a formality. The reason this audit exists is that AMWS scrutinises its site work and has never scrutinised the office. A director walking their own workplace under audit is the evidence of commitment — an audit run by the consultant alone is just another inspection.

Half one — conformity

Twelve checks. Look, do not ask. Anything that fails gets a finding, an owner and a date at the bottom of this sheet.

# Check Pass Fail N/A Notes
1 Notice board is current — the three compliance notices up, in date, not superseded ☐ ☐ ☐
2 Policies displayed — signed H&S policy on display, dated within 12 months ☐ ☐ ☐
3 Fire log up to date — alarm test, emergency lighting, last drill recorded ☐ ☐ ☐
4 Fire extinguishers and blanket in date — service label checked, not just present ☐ ☐ ☐
5 Escape routes clear, fire doors not wedged, final exits open first time ☐ ☐ ☐
6 First aid kit — contents complete, in date, burns kit present with FORM_FA_01 beside it ☐ ☐ ☐
7 First aider / EFAW cover in date and the certificate is on file, not just claimed ☐ ☐ ☐
8 Fire marshal cover in date and on file ☐ ☐ ☐
9 DSE assessments — one on file for every workstation in use, including any new starter ☐ ☐ ☐
10 Office COSHH — cleaning products labelled, stored as assessed, assessments reachable ☐ ☐ ☐
11 Records where the system says they are — pick two at random from APP_24 and go and find them ☐ ☐ ☐
12 Yard — alarm audible, housekeeping, spill kit present and unobstructed, oil storage bunded ☐ ☐ ☐

Check 11 is the one that catches things. Pick the two before you arrive, not after you have seen what is tidy.

Half two — culture

Three questions. Ask them of whoever is in. Write the answer in their words. Work them into the conversation while you walk round rather than reading out a list. There is no score.

1 · Who is the health and safety contact here, and how would you reach them?

(answer)

2 · When did a director last attend a toolbox talk or walk a job with you?

(answer)

3 · Has anyone here reported a near miss? What happened after they did?

(answer)

The question that matters most

Near-miss and close-call reporting has stood at zero for three years. That is an absence of data, not an absence of events.

If the answer to question 3 is "no" or "I don't know", ask why and record what they actually say. Whatever comes back — didn't know how, didn't think it counted, thought it would get someone in trouble, too busy, nothing happens anyway — that answer is the single most valuable line on this sheet. It is the only direct evidence AMWS has on objective H4.

Result

Tick one
🟢 Compliant — all applicable checks pass ☐
🟡 Minor findings — recorded below, to close within 30 days ☐
🔴 Major finding — a statutory duty not met or a control absent. Raise in APP_21 ☐

An audit that finds nothing has usually not been done properly. Twelve checks and three conversations in an office will usually find something.

Every 🔴 also goes into APP_21. Minor findings are tracked in APP_27 Section 5.

After the audit — three things

  1. Record the cycle in APP_27 Section 4 — date, who, result, findings. A cycle that is not in the register did not run.
  2. Raise any major finding in APP_21 with an owner and a date.
  3. Take the reporting answers to APP_22 and to the next management review. They are the evidence behind objective H4 and they are why this audit is worth 45 minutes.
