Details of Nonconformity¶
Statement of Nonconformity: OBSERVATION (rolled forward from CAR-2025-018): Worker-consultation evidence (TBT attendance template, operative questions captured during Aaron's informal site visits, the planned AMWS H&S Culture Survey (Onyx Operations) Q3 2026 rollout) is generated continuously, but no roll-up format is yet defined for inclusion in the September 2026 Management Review pack.

- Populate from May–August 2026 TBT records and site-visit actions; include in the September MR pack
Root Cause¶
Primary Cause: Worker-consultation input is captured continuously but never formatted as a discrete Management Review input. Contributing Factors: MR pack structure predates the 2026 TBT Programme and Culture Survey.
Proposed Corrective Action¶
- Create a 1-page worker-consultation roll-up template.
- Populate from TBT_PROG_01 records, APP_21 operative-raised actions, and (from Q3 2026) the AMWS H&S Culture Survey.
Verification¶

Recommendations¶
- Keep it to one page so it is sustainable.
- Carry the same template forward each MR cycle.
