Details of Nonconformity¶
Statement of Nonconformity: OBSERVATION (rolled forward from CAR-2025-001): The standing weekly Director / HSQE compliance call covers operational and stakeholder context informally and effectively, but there is no brief written capture of context updates between the formal 6-monthly APP_01 reviews, so the auditor cannot sample it.

Root Cause¶
Primary Cause: Context monitoring runs through an effective informal forum (the weekly call) but was never given a lightweight written-capture mechanism. Contributing Factors: Small-company habits — context discussed verbally, not minuted.
Proposed Corrective Action¶
- Implement a light-touch monthly context capture in the weekly-call notes.
- First evidence: the May 2026 weekly-call notes, reviewed at the next standing call.
