QEHS/NCR Ref: OBS-IA202603-01
Date: 30/04/2026
CAR ID: CAR-2026-003
Document Number: FORM_CAR001 Rev 1 ID 01/09/2025
Process/Area/Department: Support / Training & Competence
Requirement/Clause No.(s): ISO 45001:2018 - Clause 7.2
Type: ☑ Internal Audit ☐ Client Audit ☐ Inspection ☐ Complaint ☐ Sub-contractor ☐ Certification Review
Classification: ☐ Nonconformity ☑ Observation ☐ Opportunity for Improvement
Details of Observation¶
Statement of Observation: Three statutory training certificates had expired and were awaiting rebooking: Jason May Fire Marshal (EXP 15/08/2025), Leanne Mason Fire Marshal (EXP 19/09/2025), Leanne Mason Emergency First Aid (EXP 30/10/2025). AMWS retained cover on each role through other named individuals so there was no immediate operational gap, but the certificates needed to be renewed.
Planned Actions¶
Containment Action(s), including correction, with supporting completion date(s):
- Rebook all three expired statutory certificates with external providers (31/05/2026)
- File the renewed certificates in the training evidence folder
Root Cause¶
Primary Cause: Certificate currency tracking sat in a live spreadsheet held by Leanne Mason; two had already been renewed but the renewals were not yet reflected in the audit evidence folder, and one (EAW) was still to be rebooked. Contributing Factors: Director-held training records maintained separately from the operative training matrix.
Proposed Corrective Action¶
- Confirm and file the two renewed Fire Marshal certificates.
- Complete the rebooked Emergency First Aid at Work training and file the certificate on issue.
Verification¶
- [x] Jason May Fire Marshal certificate on file (completed 24/07/2025, expiry 24/07/2026, Citation).
- [x] Leanne Mason Fire Marshal certificate on file (completed 15/11/2025, expiry 15/11/2026, VitalSkills / HSQE Ltd, cert fMQmPvJqAl).
- [x] Leanne Mason Emergency First Aid at Work — rebooked, scheduled ~31/05/2026; certificate to be filed on completion.
Points noted¶
- Migrate certificate currency to ComplianceTrack so expiries surface automatically.
- Hold renewal evidence in the training evidence folder as well as the live tracker.
Feedback & Acknowledgments¶
Raised by the 2026 internal-audit cycle (IA202603). Owner and target agreed at the standing weekly Director / HSQE compliance call.

Closure¶
Corrective action (rebook the three expired statutory certificates) complete. Both Fire Marshal certificates are renewed and evidenced (Jason May 24/07/2025–24/07/2026; Leanne Mason 15/11/2025–15/11/2026). Residual item flagged on close: Leanne Mason Emergency First Aid at Work rebooked, scheduled ~31/05/2026 — certificate to be filed on completion. Operational cover was maintained throughout via other named marshals/first-aiders, so no immediate operational gap at any point.
Status: CLOSED — 26/05/2026
Closure reviewed 09/09/2026 — certificate currency since this closure
This report remains closed. The review of 09/09/2026 checked the certificates it was closed against and recorded their currency at that date: Jason May's Fire Marshal certificate had reached its expiry of 24/07/2026, Leanne Mason's runs to 15/11/2026, and the Emergency First Aid at Work certificate accepted here as "to be filed on completion" was not yet on file.
Position at 28/09/2026. Jason May's Fire Marshal certificate is renewed to 21/07/2029 and the Emergency First Aid at Work certificate is on file to 16/06/2029. Leanne Mason's Fire Marshal renewal is carried at CAR-2026-015, together with the control adopted at the same review: closure evidenced by a certificate records the expiry date, and a certificate with less than six months remaining does not close a report on its own.
