Details of Nonconformity

Statement of Nonconformity: OBSERVATION (rolled forward from CAR-2025-012): The new TBT attendance template and Aaron's informal site presence provide near-miss capture mechanisms, and the 28/04/2026 site tour produced one captured action. The next 90 days will indicate whether the near-miss reporting rate increases as the new mechanisms bed in.

Root Cause

Primary Cause: Historically low near-miss reporting rate for a small team; new capture mechanisms are in place but unproven over time. Contributing Factors: Under-reporting culture typical of small operative teams.

Proposed Corrective Action

  1. Track near-miss counts monthly for 90 days from this audit.
  2. Review at the weekly call whether the rate trends up as mechanisms bed in.
