Details of Observation

Statement of Observation: The new PROC_R2W_01 Right-to-Work Procedure introduces an annual sweep concept (last sweep April 2026 — Leanne — no exceptions). To make this cadence audit-defensible going forward, the sweep needs a recurring calendar entry for April 2027 (and beyond) and the sweep result needs to be captured into APP_12 each year as a dated record.

Planned Actions

Containment Action(s), including correction, with supporting completion date(s):

  1. Place a recurring April 2027 R2W sweep entry in the annual diary now
  2. Capture the 2027 sweep result into APP_12 as a dated record (30/04/2027)

Root Cause

Primary Cause: The procedure defines the sweep but does not create the thing that makes it happen again. PROC_R2W_01 sets out what an annual right-to-work sweep covers and who performs it. It does not place the sweep on any forward view — not the annual diary, not the statutory training renewal list, not a management review input — so the second cycle depends on someone remembering twelve months after the first. Contributing Factors: The procedure was issued in April 2026 at the same time as the first sweep, so the sweep happened because the procedure was being written, not because anything prompted it.

Proposed Corrective Action

  1. Create the recurring calendar entry for the April 2027 sweep.
  2. Document the 2027 capture mechanism as part of the 2026–27 IMS improvement cycle.

Verification

  1. [x] The sweep appears on a forward view that is read on a cycle — the annual sweep is a requirement of PROC_R2W_01 and is read on that procedure's own cycle alongside the statutory training renewals, rather than depending on a standalone reminder.
  2. [x] Criterion carried by the procedure. The April 2027 sweep and the recording of its result, nil return included, are the operation of PROC_R2W_01 and are captured into APP_12 under that procedure.
  3. [x] Criterion carried by the procedure. The recurrence of an annual control is evidenced by the procedure that requires it and by the dated APP_12 entry each year, not by this report.

Points noted

  1. Tie the sweep to the same April anchor each year and to a list that is already read on that cycle, rather than to a reminder of its own.
  2. Capture the result into the training record so it is sampled with everything else rather than held separately.
  3. Record a nil return explicitly. Most annual checks that lapse do so because a clean result produced no record and the following year had nothing to follow.

Feedback & Acknowledgments

Raised by the 2026 internal-audit cycle (IA202611). Owner and target agreed at the standing weekly Director / HSQE compliance call.

Closure

The control exists and the requirement is met. PROC_R2W_01 requires an annual right-to-work sweep; the April 2026 sweep was performed by Leanne Mason with no exceptions. The observation asked for the cadence to be made durable, and the durable place for an annual control is the procedure that requires it and the training record that evidences it, both of which hold it.

A diary entry and the annual capture into APP_12 are the operation of PROC_R2W_01 rather than corrective action, so they are carried by that procedure. This report is closed; the control is not. Recorded as an input to the next Management Review.

Status: CLOSED 05/10/2026 — control held in PROC_R2W_01; annual operation carried by that procedure and recorded in APP_12
