Details of Nonconformity

Statement of Nonconformity: OBSERVATION (rolled forward from CAR-2025-011): APP_16 Emergency Preparedness & Response Matrix and SOP 5.4 are clear, and spill kits are in van packs and in date. What is missing is a formal scenario-based environmental drill calendar — fuel spill on highway, hydraulic leak at depth, contaminated-ground discovery during excavation — with documented lessons.

  1. Hold one scenario-based environmental emergency drill before the September 2026 Management Review (30/09/2026)
  2. Capture outcomes into APP_16 testing log and APP_21

Root Cause

Primary Cause: Environmental emergency arrangements are documented but the practical drill schedule was never formalised. Contributing Factors: Drills happen reactively around real incidents rather than on a calendar.

Proposed Corrective Action

  1. Pick one scenario (e.g. fuel spill on the highway) and run a documented drill.
  2. Record lessons into the APP_16 testing log; raise any actions in APP_21.

Recommendations

  1. Rotate the scenario each year from the APP_16 matrix.
  2. Pair with the BCP desktop where practical to save coordination effort.
